THE PACKAGE THAT YOU GIVE THE REGIONAL V.P.  SHOULD CONTAIN THE FOLLOWING ITEMS. PLEASE SORT THEM ACCORDING TO EACH ITEM, NOT BY INDIVIDUAL PLAYERS.  MOST OF THE FORMS ARE IN OUR "FORMS" TAB ON THE LEFT.

 a.     Your ROSTER from Gotsoccer.com

b.     Copy of Club roster that administration will give you that verifies that each player on your team has paid. You will no longer be required to provide me with a copy of the GFSC registration form. Players not on this list will have passes withheld until proof of payment is shown.

c.      Signed passes, with no white outs, with pictures attached or pre-loaded (preferred),

d.      Non-Volunteer (trainer) copy of pass - front and back.

e.      Medical release forms.

f.       MNJYSA Player Membership form.

g.      Coach Membership form

h.      Coach Kidsafe form

i.        S.A.G.E. form signed
